Surgery often involves charges from more than one provider, and insurance coverage varies by plan. The answers below cover the questions we hear most. If anything about your bill or coverage is unclear, call us at (801) 277-2062 and our staff will walk you through it.
Billing questions
Why am I receiving more than one bill for the same surgery?
There are a number of separate charges associated with your surgery. You may receive bills from several providers, including:
- Our surgery center: the facility fee for your surgery
- Your surgeon's office: the fee for performing your surgery
- Anesthesiologist: the fee for anesthesia services
- Radiology: fee for x-ray, if applicable
- Pathology: fee for tissue specimens requiring further examination, if applicable
- Laboratory: fee for tests run on specimens removed during surgery, if applicable
What forms of payment do you accept?
We accept cash, check and all major credit cards, including Visa, Mastercard, Discover and American Express.
When should I expect my first bill?
We will bill you for any balance due after your insurance company has paid your claim. There may be a delay if your insurance company paid incorrectly and we have resubmitted your claim for a corrected payment.
Can I set up a payment plan?
We ask that all balances be paid in full within 90 days. If you encounter problems paying within 90 days, please contact our office and ask for the billing department.
What is your credit and payment policy?
We will submit insurance claims for you. We may request that your deductible and copay amounts be paid on or before your date of surgery. You will receive a notice from us regarding the amount to pay.

What insurance plans do you accept?
We accept most forms of insurance. Please contact us at (801) 277-2062 with any questions about your coverage.
Will you verify my insurance before surgery?
Yes. When our staff contacts you before your procedure, we confirm your insurance information and let you know of any expected deductible or copay amounts.
Cash pay pricing
Physicians Surgery Center believes in being transparent in its pricing. For self-pay patients, we publish cash pay costs for common procedures so you can understand prices in advance of your procedure:

A few important notes about this pricing:
- Prices are estimates for the facility fee only. They do not include professional fees (surgeon, anesthesiologist, pathologist, radiologist and other independent practitioners), implants, high-cost drugs or secondary procedures.
- Cash pay pricing applies to self-pay patients who have pre-scheduled their service. It does not apply to patients with Medicare, Medicaid, other government programs or commercial insurance.
- For bilateral or multiple-level procedures, the first procedure is payable at 100% of the cash pay price, the second at 50%, and the third and any additional at 25%. Implants and other supplies are payable at cost plus 10%.
- Actual prices on your final bill may vary based on your medical condition, complications, final diagnosis and the treatment ordered by your providers.
